A murder-suicide in a northern Californian hospital leads to an investigation by the on-call doctor, which reveals a plot by an insane toymaker to kill as many people as possible on October 31st through an ancient Celtic ritual and deadly Halloween masks.

The fourth installment in the Halloween franchise follows the unstoppable psychopath Michael Myers returning to his hometown once again on Oct 31st, this time to kill his niece. Donald Pleasence returns also as the Michael's psychiatrist who is hellbent on stopping him.

After lying in a coma for a year, Michael Myers awakens and stalks his way back to his hometown with the intent again on killing his niece, Jamie (Danielle Harris), who has been confined to a mental institution since Michael's last attempt to slay her. Suspecting a psychic link between Michael and Jamie, psychiatrist Dr. Sam Loomis (Donald Pleasence) attempts to stop Michael's latest rampage.

In 1949 composer Roman Strauss is executed for the murder of his wife. In 1990s Los Angeles, a detective finds out that, a mute amnesiac woman, under hypnosis, is somehow linked to the Strauss murder.

Five years after his wife, Joan (Carrie Snodgress, THE FURY), had him wrongfully and deliberately committed to a mental institution so she could live with her boyfriend (David Carradine, KILL BILL, TV’s KUNG FU), Malcolm (Peter Jason, 48 HRS) escapes on Halloween night and arrives back at his old house to murder her. Except Joan is not home - she’s out with her boyfriend while babysitter Linda (Jacqueline Giroux, THIS IS A HIJACK) is looking after Malcolm’s 10-year-old son Christopher (Chris Graver), an equally psychotic and demented practical joker until, under cover of darkness, Malcolm shows up and begins a real terror game of attrition with Linda.
Steve Railsback (THE STUNT MAN, LIFEFORCE), Paul Bartel (EATING RAOUL), Jillian Kesner (RAW FORCE), John Blyth Barrymore, Football greats Dan Pastorini and Tim Rossovich co-star.
